1 Proceed to the Mun. Assesor’s Office for consultation of the transaction Provide the client the requirements for land to be declared new Mun. Assessor For titled property (2 copies each)
a. A certified true copy of free patent, homestead of  miscellaneous sales application must be submitted
b. A certified true copy of the title issued by the Register of Deeds, certifying among others, that the original copy of which is intact and existing in the said registry
c. Approved survey plan
for untitled property (2 copies each)
a. A survey plan prepared by a duly licensed Geodetic Engineer duly approved by the Land Management Bureau (LMB) of the Department of Environment and Natural Resources
b. A certification from the Community Environment and Natural Resources Office (CENRO), stating among others, that the land is within the alienable and disposable area
c. An affidavit of ownership and/or sworn Statement declaring the Market Value of Real Property filed by the owner/ administrator; Affidavit that the applicant is in long, continuous and notorious possession of the property
d. A certification from the barangay captain that the declarant is the present possessor and occupant of the land and the certification of the adjoining owners duly sworn to by the barangay captain and/or the municipal mayor.

2 Submit the necessary requirements for checking Receive and review the documents submitted Mun. Assessor     10 min.
3   If documents are complete, conduct ocular inspection/ investigation report on the land to be declared new Mun. Assessor & Chito Lorete

4   Let the client pay the processing fee at the Treasurer’s Office Mun. Assessor Processing Fee 150.00 (processing fee)  
5 Go to the treasurer’s Office & pay the processing fee          
6 Proceed back to the Mun. Assessor’s Office and submit Official Receipt for Processing fee Receive the OR for processing fee and let the client sign on the tax declaration form and the sworn statement affixing the residence certificate No. including date and place of issuance. 7 Wait for advise when to follow-up the tax declaration Advise the client to follow-up after two (2) weeks from submission Mun. Assessor     2 min.
8   Process the appraisal and assessment of the land on the FAAS Mun. Assessor     2 hrs
